Becoming a Client
God's Love We
Deliver offers home delivered meals and nutritional counseling and
education for the seriously ill who are physically limited in their
activities of daily living. If you have questions about your eligibility,
please click here. We also
provide meals to any dependent children 17 and younger that are
under your care and to senior caregivers 60 or over if you are also
a senior.
Nutritional
counseling and education is included in our meal delivery program.
To refer
yourself to God's Love We Deliver, please follow these instructions:
- Call our
Client Services Department at 212-294-8102 or 1-800-747-2023 and
identify yourself as a potential client. A staff person will determine
your eligibility and schedule a time to do a telephone intake
interview. Please have your primary physicians name and number
available to give to the Client Intake Specialist.
- If you reach
our voicemail, press "1" and leave a message with your
name (spelling if necessary), your telephone number, your zip
code and a good time to reach you.
- Please note:
due to our heavy call volume, many calls go directly to voicemail.
We check these messages on a daily basis and will gladly return
your call as soon as possible.
Allow up to
25 minutes for the phone interview and review how to prepare
for your intake. Clients must obtain a valid God's Love We Deliver
medical referral form that must be completed by his/her medical
provider within 10 business days of completing a phone interview
with our staff to continue to receive meals.
To refer
a family member, friend or neighbor to GLWD, take the following
steps:
- Have the
person with you when you call. If you reach a staff person, we
need to speak to the potential client to receive a verbal release.
If you reach the voicemail, please have the potential client leave
their name, number and the best time to reach them on the message.
- OR fax a
signed Release
of Confidentiality Form to 212-294-8198.
- Please note:
leaving someone else's name and number on our voicemail does not
enable us to speak to the potential client. Only with a verbal
or written release from the potential client can we speak to them.
