Referring
a Client
God's Love We
Deliver offers home delivered meals and nutritional counseling and
education for the seriously ill who are physically limited in their
activities of daily living. If you have questions about your client
or patient's eligibility, please click
here. We also provide meals to any dependent children 17 and
younger that are under your client or patient's care and to senior
caregivers 60 or over if your client or patient is also a senior.
We are unable
to include in our eligibility criteria individuals that may have
an inability to shop and cook meals for themselves due to poverty,
mental illness, his/her advanced age and frailness, or an injury,
chronic disease or physical syndrome he/she has had since birth.
If your client does not meet our eligibility criteria, we will gladly
refer you to one of our affiliate agencies or to a Community
Partner.
To refer
your client to God's Love We Deliver meal and nutritional counseling
services:
- Complete
the referral packet found on the Forms
Link. This includes: HIPAA Release Form signed by your client,
releasing medical information to GLWD; Medical Eligibility Form
signed by your client's primary care physician; and GLWD Referral
form (optional). Fax these forms, with your name and contact number,
to the Client Services Department at 212.294.8198. We will then
contact the client, or you, as indicated.
- Call Client
Services at 212.294.8102 or 800.747.2023 with your client. Identify
yourself and what agency you are calling from. Indicate that you
are with a potential client. A staff person will then ask to speak
to this person, obtaining a verbal release. From there, the staff
person will determine your client's eligibility and schedule a
time to do a telephone intake interview. Please have your client's
primary physicians name and number available to give to the Client
Intake Specialist.
- If you reach
our voicemail, press "1" and leave a message with your
name (spelling if necessary), your telephone number, and your
agency. Then state that you are calling to refer your client and
put them on the phone. They will then leave their name (spelling
if necessary), telephone number, zip code and a good time to reach
them. If they wish for you as the referral source to handle the
intake interview, please have them indicate that in the message.
Please note:
leaving someone else's name and number on our voicemail does not
enable us to speak to your client. Only with a verbal or written
release from the potential client can we speak to them.
