About the Role

The Business Development & Community Partnerships Department establishes contractual relationships and partnerships with managed care providers, health systems and community-based organizations in an effort to engage and serve individuals in need of our medically tailored home-delivered meals. The Community Outreach Associate is responsible for conducting outreach, engagement and education sessions to cultivate increased and consistent referrals from various types of providers. The role is specifically responsible for supporting the team’s efforts in increasing the community’s awareness of God’s Love, the number of clients and members served from existing and new partnerships, and the number of healthcare and community partnerships we have. The person in this role will use research findings to engage partners, build relationships, conduct outreach to new and current partners, and ensure that the organization is attracting client referrals to meet annual goals, including the target number of clients served and annual revenue.

  • This position is a hybrid position, allowing for 1 remote day.  
  • This is a Full-Time exempt position, paying $66,500 annually.

Essential Duties and Responsibilities:

  • Conduct outreach and engagement with all current and potential partners by conducting meetings and presentations to managed care organizations, healthcare systems, federally qualified health centers, community-based organizations, community groups and others.
  • Create, strengthen, and execute the outreach, engagement and education plan to reach all current and potential referrers to the philanthropic program including Ryan White-funded programs for People Living with HIV and other grant-specific initiatives as well as to healthcare and managed care contracted partners.
  • Contribute to the organization’s actions to support the achievement of health equity in services by developing and executing a strategy to reach individuals in historically marginalized communities with high incidence of chronic disease.
  • Actively demonstrate strong customer service, organizational and presentation skills. Support the team’s efforts in providing customer service support to organizations, providers and clients to maintain relationships with partner agencies.
  • Work closely with the Chief Business Development Officer, colleagues in the Business Development & Community Partnerships Department and other client-facing Managers/Directors on a strategy to cover all outreach activities. In this, coordinate tasks and track meetings and presentations conducted by the department.
  • Maintain consistent and engaged contact with staff at community-based and partnering organizations to establish education sessions and teleconferences for current and new staff at agencies. Engage with providers regarding referral trends and programmatic and service updates. Create presentations (both online and in-person) and materials to foster increased understanding of the importance of medically tailored meals and medical nutrition therapy for individuals living with chronic illness.
  • Work closely with the Manager of Healthcare Partners and Manager of Client Services to facilitate new enrollments that occur as a direct result of outreach efforts.
  • Keep records of outreach and marketing activities that includes trends, areas of potential growth and challenges for stakeholders and prepare database and project reports both internally and externally. Responsible for tracking and reporting on activities which includes entering all relevant partner/contact details into Monday.com, NXT/Raiser’s Edge Database.
  • Support agency advocacy efforts (such as for Food is Medicine public policy) that relate to partnership and contracting opportunities such as those for New York State-specific Medicaid Redesign Initiatives.
  • Work with the Communications team members to create collateral materials, announcements, blog posts and press releases for contracted partnerships.

In addition, you meet the following requirements:

  • Bachelor’s degree with at least 3 years of fast-paced healthcare or nonprofit experience. Familiarity with the NYC community-based and/or healthcare environments is a plus.
  • Experience conducting community outreach and/or engaging in community-based work in a healthcare or CBO setting
  • Experience engaging effectively and persuasively with a diverse audience of community members and other external stakeholders
  • Superior attention to detail and strong administrative skills
  • Excellent interpersonal communication, and public speaking skills
  • Must be able to operate effectively as part of a team as well as independently
  • Strong project management skills
  • Superior time management skills with the ability to work effectively under deadlines
  • Strategic mindset with an ability to think and focus on both the near- and long-term goals
  • Available evenings and weekends for outreach and engagement events as needed
  • Microsoft Office proficiency
  • com, Canva, Raiser’s Edge NXT experience a plus
  • Bilingual (Spanish/English) ability preferred
