Who is involved with the 1115 Waiver?
The NYS 1115 Waiver will open the door for God’s Love We Deliver and all community-based organizations in the 14 regions where the State is proposing to adopt the Waiver.
The 1115 Waiver enables community-based organizations (CBOs), such as God’s Love We Deliver, to join a social care network (SCN). CBOs are responsible for delivering high-quality, evidence-based, and accessible social care services. The network will be administered by a lead entity that will coordinate the delivery of social care services by CBOs to Medicaid members with shared data & technology between the SCN, CBOs, NYS, and other stakeholders, to help evaluate the impact on health outcomes.
Medicaid recipients will be screened by a SCN to determine if they qualify for programs to address their health-related social needs. Our food and nutrition services will be available to qualifying Medicaid recipients.
What is provided through the 1115 Waiver?

- Nutrition reassessment after a six-month period
Where does the 1115 Waiver apply?
God’s Love will be able to serve the regions in the New York City area that are covered under the 1115 Waiver:
- Region 1: Capital Region (Albany, Columbia, Greene, Rensselaer, Montgomery, Saratoga, Schoharie)
- Region 2: Western New York (Cattaraugus, Chautauqua, Erie, Niagara)
- Region 3: Hudson Valley (Dutchess, Orange, Putnam, Rockland, Sullivan, Ulster, Westchester)
- Region 4: Bronx
- Region 5: Brooklyn
- Region 6: Queens
- Region 7: Manhattan
- Region 8: Richmond
- Region 9: Finger Lakes Region (Allegany, Cayuga, Chemung, Genesee, Livingston, Monroe, Ontario, Orleans, Schuyler, Seneca, Steuben, Wayne, Wyoming, Yates)
- Region 10: Southern Tier (Broome, Chenango, Delaware, Otsego, Tioga, Tompkins)

When will God's Love be able to deliver meals to new clients based on the 1115 Waiver?
God’s Love is stepping up its technological and physical infrastructure to support the launch of the 1115 Waiver. When the SCNs are ready to launch, so are we!
In late 2024, God’s Love will begin to serve those who are eligible to receive our services through the 1115 Waiver.
Why is God's Love delivering to more people through the 1115 Waiver?
As our services can expand through the 1115 Waiver, God’s Love becomes more able to fulfil its mission of delivering to all who can use our services. God’s Love provides Medically Tailored Meals and medical nutrition therapy to those living with severe illness and who are too sick to cook or shop for themselves.
How will God's Love work with SCNs to deliver to its clients?
As the processes for providing our services to New Yorkers across the state are implemented, each SCN will develop their own workflow. We’ll work with SCNs on providing our Medically Tailored Meal services to their members. Our processes of onboarding new clients, including our nutrition screening assessment, are unique in New York state and is the only service of it’s kind in New York City.
