Happy National Public Health Week! For 40 years, God’s Love We Deliver has worked to improve the lives of New Yorkers affected by illness. Beginning with our founder, Ganga Stone, delivering a meal to a friend dying from AIDS, God’s Love has grown to serve over 200 diagnoses on our medically tailored meal program. Now, we are proud to partner with the New York State Social Care Network to provide Medicaid recipients with additional supportive services.
At no cost to Medicaid members, our Community Health Workers (CHWs) are dedicated to connecting members to programs. Following a screening, we assist eligible Medicaid members with a wide variety of social services, including nutrition, transportation for non-medical needs, housing support, home remediation for those with Asthma, and job & education connections. Our CHWs always follow up to ensure they’re getting proper care and reach out to service organizations on members’ behalf to address any questions or concerns. We also have bilingual team members and work with live translation services to ensure we can communicate with all our community members.
Our primary mission at God’s Love will always be nutrition. These additional supportive services provide New Yorkers with crucial public health resources to improve their daily lives. More importantly, these early interventions addressing social needs significantly improve long-term public health! We’re proud to celebrate National Public Health Week with a commitment to connect our community to the services they need for their health journey!
To get started, members visit www.glwd.org/SocialCare and complete the online self-screener. Next, a CHW at God’s Love will call you for an assessment. Qualifying conditions for services include chronic Asthma, Home Health enrollees, those who have visited the emergency room five times or more in the last 6 months, those newly released from prison, and individuals with Serious Mental Illness, Intellectual & Developmental Disabilities, or Substance Abuse Disorder.
