Partnering with the Mount Sinai Health System and Mount Sinai Performing Provider System Agencies

We are honored and excited to be working with the staff from Mount Sinai St. Luke’s and Mount Sinai Jackson Heights to provide our medically-tailored meals to individuals that are diagnosed with severe diabetes and that have had several hospitalizations in the past year.  The goal of the pilot is to provide nutritional stability, increase comfort, and reduce hospital utilization for participants.

The partnership is a six-month pilot that involves enrolling fifteen high-need Medicaid patients that will be enrolled on our program and receive nutritional assessments and counseling from our team of registered dietitians.  God’s Love team of registered dietitian nutritionists will develop a nutritional assessment with each enrolled client and tailor meals to address each patient’s needs. These meals include delivering breakfast, lunch and dinner for six months.

“We’re so excited to be in this new partnership with Mount Sinai as well as the opportunity to work with the staff at both Mount Sinai St. Luke’s and Jackson Heights to see what the impact of medically tailored meals can have on those individuals dealing with severe diabetes,” said Dorella Walters, Senior Director of External Program Affairs.

We’ve worked with Mount Sinai Health System for more than thirty years and receive referrals from several of their departments across the city.  This new opportunity has only strengthened the collaboration between both organizations as our goal is to transform healthcare through the provision of medically tailored meals.

With our Community Partners Program, God’s Love receives reimbursement through contracts with MLTC plans, helping to create long-term sustainability for our clients. This also helps position God’s Love as a nutrition leader providing health services to long-term care populations.

Of course, food is more than just medicine. Food is love. A highlight of the Expo came when our Director of Culinary Services, Daniel Metzger, shared samples of our meals with staff from agencies that refer their clients to us. It warms our hearts when healthcare providers are able to see—and taste—the love that goes into every meal at God’s Love.
